Extracurricular Form 2026

The Extracurricular Activity Form allows your child to participate in additional activities/programs while they are registered and attending summer camp.

Eligibility: Only programs taking place at the Johnston Recreation Center, Oasis pool, or Circle Park during summer camp hours are eligible. Wednesday’s activities are not conducive with camp field trips, please plan accordingly. Top Dog field trips vary, please check your calendar. If your child’s activity starts at 8:45am, you must drop them off there.

Parents: Please inform the leader of the extracurricular program/activity that your child will be escorted by a summer camp staff.

Submit your form via email to [email protected] or in person at the JRC front desk.
(24 hours notice needed for accommodations.)
One Form per activity/session is required
