Walking Home Permission Slip 2026

This form grants permission for your child to walk to and from day camp independently. It also acknowledges parent responsibility for pickup during inclement weather and outlines the potential need to use and pay for P.M. Sunset Extended Care if no approved adult is available for pickup.

Eligibility: Ages 10 and Up
